WHAT is it? - - - The Patient Centered Medical Home (PCMH) is a way to manage and organize your health care with your primary care provider. We can help by serving as the coordinator of your care and want to know of any changes in your health.

- At NCHC we work as a team to meet your needs.
- We coordinate your care from check-ups to specialized treatment.
- We provide care that is evidence based (based on research). We want to help prevent health problems, not just treat them, so your provider will review your prevention needs too.
- You are the center of the team. We want to help you take control of your health and will help you choose and accomplish the goals that are right for you.
- We want you to receive the services you need when you need it. We keep open a limited number of appointments each day for sick visits.
- To help us provide better care, please be prepared to provide us with a complete medical history, and inform us of any health care received outside of NCHC clinics.
