Your Medical Information

Medical Record Information

You can rest assured that your medical information and records are kept confidential according to current law.

When being seen at one Northland Health Center’s clinics, your provider may ask for access to your medical records from other doctors you have seen. In doing so, they may ask you to sign a Medical Record Release form. This will allow us to ask other doctors to forward your medical information to us.  If you already have your medical records, please bring them with you to your appointment and we will place a copy in your electronic medical record.

If you need Northland Health Center to send your records to another healthcare provider, you will be asked to sign a Medical Record Release form that will authorize us to forward your medical record on your behalf.

Please allow up to ten business days for your records to be received.

Brent went into NHC for a regular checkup. His routine test results came back slightly elevated. He felt healthy and had no concerning symptoms, so he didn’t think anything of it. Despite this, his NHC Provider encouraged him to repeat the test. When the results came back slightly more elevated, our Provider scheduled Brent to have further testing done to rule out any issues. He then was scheduled to have a biopsy, and it was discovered that Brent had prostate cancer. Brent endured 5 days a week of radiation for 9 weeks. Brent is cancer free today and he credits NHC for saving his life. He is immensely grateful for the affordable cost of his services, and even more so to the Providers and staff at NHC. “I only have positive remarks to say about Northland Health Centers,” Brent says. “They’ve led me in the right direction.” Going forward, Brent plans to continue his care at NHC and trusts that we will continue to make a positive impact for him, his family, and his community.
